Endorsement in Event of Death/Absent of Payee 400-19-120-95

In the event of the recipient’s or protective payee's death or absence before an issued check has been properly endorsed, the check can be endorsed for one month in the following manner, in order of preference:
- By the surviving spouse, if such person has been living with the payee; or
- By the director of the county social service office, if there is no surviving spouse.
Whenever one of the preceding methods of endorsement is used, the completed and signed statement below must appear on the back of the check immediately following the endorsement:

Checks not so endorsed will be returned when presented for payment.
A payment or any portion of the payment not needed shall be promptly returned to the Department of Human Services, Fiscal Administration.
